"If one parent or guardian cannot apply with their child, submit Form DS-3053 with the child's passport application. The parent that cannot apply with the child must:

Sign and date Form DS-3053 in the presence of a certified notary public, and
Submit a photocopy of the front-and-back of the photo ID that they provided to the notary public.

공증인 앞에서 DS-3053 양식에 서명하고 공증인에게 제공한 사진이 부착된 신분증의 앞면과 뒷면 사본을 제출해야 하며
DS-3053 공증을 받을 때 일부에서는 일반 공증은 안되고 미국 대사관이나 영사관에서 공증을 받아야 한다고 되어있네요..
